Job summary

The Salvation Army in Nebraska is seeking an experienced case manager to promote community stabilization through home and community-based services. You will perform in-home/mobile case management addressing illness management, peer support, crisis prevention/intervention, and resource utilization.

Responsibilities include developing ISP, conducting assessments, coordinating services, and providing crisis intervention and discharge planning.

Qualifications

• High school diploma or equivalent required; plus two years lived experience or direct care in human services.
• Experience with case management and developing ISP plans.
• Ability to use standard office software and maintain accurate records.

Responsibilities

• Complete comprehensive strength-based psychosocial assessment within 7 days of admission.
• Develop ISP integrating strengths and needs with measurable goals and interventions.
• Maintain supervision of ISP and ongoing assessments including risk and safety plans.
• Schedule and attend minimum six face-to-face contacts per month; more as needed.
• Monitor medication compliance and assist with self-medication structuring.
• Provide family education and support with problem solving and crisis intervention.
• Ensure engagement in other mental health treatment services as recommended.
• Assist in transitioning to lower level of care and greater independence.
• Coordinate access to medical, social, housing, transportation and other services.
• Report progress to treatment team and document outcomes and plans.
• Respond to crises; transition consumer back to community after discharge when needed.
• Complete required case files and maintain statistics and reports.
• On-call coverage rotation and in-office response as warranted.
• Driving is an essential function of this position.
• All other duties as assigned.
