Job summary

The Salvation Army in Omaha, NE is seeking a full-time case management professional to support community stabilization through in-home and mobile services. You will perform need-based assessments, develop and monitor service plans, and coordinate with families and supports while following trauma-informed practices.

Qualifications

• High school diploma or equivalent required.
• Two years lived experience or two years direct care experience in a human service field or two years training in human services or a bachelor’s degree in a human service field.

Responsibilities

• Complete comprehensive strength-based psychosocial assessment within 7 days of admission.
• Develop ISP with goals and interventions within 7 days of admission.
• Maintain ISP and ongoing assessments including risk assessments and safety contracts.
• Schedule a minimum of six face-to-face contacts or six hours of direct contact per month.
• Coordinate services across medical, social, housing, transportation and other supports.
• Support transition to lower level of care and greater community independence.
• Provide crisis intervention and linkage to community resources as needed.
• Maintain case files, documentation, and reporting as requested by Program Director.
