Job summary

ReDiscover is seeking a Community Support Specialist to join the CAP Program in Raytown, MO. You will maintain a caseload of adults with severe mental illness, provide on-site and community-based supports, and help clients achieve long-range goals in symptom management, employment, and independent living.

This role requires a 4-year degree or equivalent experience, a valid driver's license and reliable vehicle, and strong communication and customer service skills.

Qualifications

• 4 year degree or 4 years of equivalent experience in lieu of degree.
• A valid automobile driver's license, appropriate insurance, and a safe, reliable vehicle available for use during work hours for transporting clients.
• At least two years of clinical experience in direct service to children/adolescents in a mental health setting preferred.
• Knowledge of Kansas City metro community resources and ability to establish partnerships with community partners.
• Excellent customer service skills with a recovery focus.
• Strong written, oral and interpersonal communication skills.
• Intermediate computer skills (typing/keyboard) and familiarity with Word and Excel.

Responsibilities

• Engage difficult-to-serve clients by researching resources to gain access and connections.
• Remove barriers by helping clients participate in community or social services and referring to educational or work programs.
• Assist clients with transportation barriers identified in their treatment plan, including valid use of a personal vehicle.
• Collect and document client information in ReDiscover’s electronic medical record.
• Collaborate with an internal multidisciplinary team to coordinate client care and wellness.

Clients receive medically necessary on-site services in a clustered apartment setting because of a history of failure in multiple community settings and/or the presence on an ongoing risk of harm to self or others which would otherwise require long term psychiatric hospitalization.**What You’ll Do*** Maintain and interact with a caseload of clients with diverse characteristics either in-person or over the phone.* Facilitate each client’s achievement of long range outcomes of symptom management, employment and independent living based upon the needs of the client.* Engage difficult to serve clients by investigating all potential resources in order to gain access and make connections.* Help remove barriers for clients, which may include helping clients participate in community or social service programs and referring clients to educational or working programs.* Assist clients with transportation barriers as identified in their treatment plan.
