ReDiscover, a nonprofit community mental health center serving southeastern Jackson County, Missouri, seeks an Administrative Community Support Specialist for the CAP program to manage caseloads, coordinate medication-related care, and assist clients with transportation and documentation.

With a 4-year degree or equivalent experience, you will collaborate with a multidisciplinary team to connect clients with resources, ensure timely communication, and promote recovery-focused, patient-centered

Qualifications

  • 4 year degree or 4 years of equivalent experience in lieu of degree.
  • A valid automobile driver's license and reliable car.
  • 2+ years clinical experience in direct service to children/adolescents in a mental health setting preferred.
  • Knowledge of the Kansas City metro area resources and health/social service systems.
  • Excellent customer service skills with a recovery focus.
  • Strong written, oral and interpersonal communication skills.
  • Intermediate computer skills (Word, Excel, email).

Responsibilities

  • Maintain and interact with a caseload of clients with diverse characteristics either in-person or over the phone.
  • Facilitate each client’s achievement of long range outcomes of symptom management.
  • Engage difficult to serve clients by investigating all potential resources to gain access and connections.
  • Coordinate medication-related care between clients, ReDiscover providers, outside medical providers, pharmacies, Housing staff, and the Housing Nurse to ensure continuity of care and timely communication.
  • Maintain accurate medication records and MARs, including updating medication lists and orders, resolving discrepancies, and reflecting current medications.
  • Coordinate medication delivery and receipt across Housing locations, including pharmacy drop-offs, picking up medications, and collaboration with Housing Nurse.
  • Assist clients with transportation barriers as identified in their treatment plan.
  • Collect and maintain information about clients in ReDiscover’s designated electronic medical record.
  • Communicate and collaborate with an internal multi-disciplinary team to address wellness and coordination of client care.

Clients receive medically necessary on-site services in a clustered apartment setting because of a history of failure in multiple community settings and/or the presence on an ongoing risk of harm to self or others which would otherwise require long term psychiatric hospitalization.What You’ll DoMaintain and interact with a caseload of clients with diverse characteristics either in-person or over the phone.Facilitate each client’s achievement of long range outcomes of symptom management.Engage difficult to serve clients by investigating all potential resources in order to gain access and make connections.Coordinate medication-related care between clients, ReDiscover providers, outside medical providers, pharmacies, Housing staff, and the Housing Nurse to ensure continuity of care and timely communication regarding medication needs, changes, and concerns.Maintain accurate medication records and MARs, including obtaining updated medication lists and orders, identifying and resolving discrepancies, and ensuring documentation reflects clients’ current prescribed medications.Coordinate medication delivery and receipt across Housing locations, including being available for pharmacy drop-offs, picking up medications when necessary, checking medications in according to established procedures, and collaborating with the Housing Nurse and treatment team regarding any concerns or follow-up needs.Assist clients with transportation barriers as identified in their treatment plan. This includes transporting clients in the employee's personal vehicle to various appointments and area resources.Collect and maintain information about clients and document interactions within ReDiscover’s designated electronic medical record.Communicate and collaborate with an internal multi-disciplinary team to address various aspects of wellness and ensure positive communication regarding the coordination of client care.Requirements of the Position4 year degree or 4 years of equivalent experience in lieu of degree.A valid automobile driver’s license, appropriate automobile insurance and a safe, reliable, registered automobile available for use during work hours which includes transporting clients in personal vehicle.At least two (2) years clinical experience in a direct service provision to children and adolescents in a mental health setting preferred.Knowledge of the community resources as well as health and social service systems in the Kansas City metro area and skill in establishing working relationships with community partners preferred.Excellent customer service skills with a recovery focus.Strong written, oral and interpersonal communication skills.Intermediate computer skills in typing/keyboarding, Microsoft Word and Excel programs as well as the use of email.Grow With UsWe’re committed to your professional development and will support your growth as a Community Support Specialist through training, collaboration, and meaningful work.Ready to join a team that’s passionate about people?
