Job summary

ReDiscover, a nonprofit community mental health center serving southeastern Jackson County, Missouri, seeks an Administrative Community Support Specialist for the CAP program to manage caseloads, coordinate medication-related care, and assist clients with transportation and documentation.

Qualifications

• 4 year degree or 4 years of equivalent experience in lieu of degree.
• A valid automobile driver's license and reliable car.
• 2+ years clinical experience in direct service to children/adolescents in a mental health setting preferred.
• Knowledge of the Kansas City metro area resources and health/social service systems.
• Excellent customer service skills with a recovery focus.
• Strong written, oral and interpersonal communication skills.
• Intermediate computer skills (Word, Excel, email).

Responsibilities

• Maintain and interact with a caseload of clients with diverse characteristics either in-person or over the phone.
• Facilitate each client’s achievement of long range outcomes of symptom management.
• Engage difficult to serve clients by investigating all potential resources to gain access and connections.
• Coordinate medication-related care between clients, ReDiscover providers, outside medical providers, pharmacies, Housing staff, and the Housing Nurse to ensure continuity of care and timely communication.
• Maintain accurate medication records and MARs, including updating medication lists and orders, resolving discrepancies, and reflecting current medications.
• Coordinate medication delivery and receipt across Housing locations, including pharmacy drop-offs, picking up medications, and collaboration with Housing Nurse.
• Assist clients with transportation barriers as identified in their treatment plan.
• Collect and maintain information about clients in ReDiscover’s designated electronic medical record.
• Communicate and collaborate with an internal multi-disciplinary team to address wellness and coordination of client care.
