Job summary

ReDiscover is seeking a Community Support Specialist for its CPRC program in the Kansas City area. You will manage a diverse client caseload, support symptom management, employment, and independent living outcomes, and help clients access community resources.

The role requires a 4-year degree or equivalent experience, a valid driver’s license, and strong communication and computer skills. You will work with a multidisciplinary team to coordinate care and remove barriers to services.

Qualifications

• Bachelor's degree or 4 years of equivalent experience.
• Valid driver’s license and reliable vehicle for client transport.
• 2+ years clinical experience in a direct service provision to children and adolescents in a mental health setting preferred.
• Knowledge of the Kansas City metro area resources and health/social service systems.
• Excellent customer service with a recovery focus.
• Strong written, oral and interpersonal communication skills.
• Basic computer skills: typing, Word, and Excel.

Responsibilities

• Maintain and interact with a caseload of clients in-person or over the phone.
• Facilitate each client’s achievement of long-range outcomes of symptom management, employment and independent living.
• Engage difficult-to-serve clients by investigating resources to gain access and make connections.
• Help remove barriers for clients by connecting to community or social service programs and referrals.
• Assist clients with transportation barriers as identified in their treatment plan, including driving in a personal vehicle.
• Collect and maintain information about clients and document interactions in ReDiscover’s EMR.
• Collaborate with an internal multi-disciplinary team to coordinate client care.

About the CPRC Program

CPRC is a case management focused program that provides individuals with support to live an independent life in the community. Through a variety of multidisciplinary services that clients can choose from including medication management, therapy, group therapy, health and wellness support, employment services and transportation support, clients can develop skills, access resources and learn ways to manage their emotional and physical health.

What You'll Do

• Maintain and interact with a caseload of clients with diverse characteristics either in-person or over the phone.
• Facilitate each client's achievement of long range outcomes of symptom management, employment and independent living based upon the needs of the client.
• Engage difficult to serve clients by investigating all potential resources in order to gain access and make connections.
• Help remove barriers for clients, which may include helping clients participate in community or social service programs and referring clients to educational or working programs.
• Assist clients with transportation barriers as identified in their treatment plan. This includes transporting clients in the employee's personal vehicle to various appointments and area resources.
• Collect and maintain information about clients and document interactions within ReDiscover's designated electronic medical record.
• Communicate and collaborate with an internal multi-disciplinary team to address various aspects of wellness and ensure positive communication regarding the coordination of client care.

Requirements of the Position

4 year degree or 4 years of equivalent experience in lieu of degree.

• A valid automobile driver's license, appropriate automobile insurance and a safe, reliable, registered automobile available for use during work hours which includes transporting clients in personal vehicle.
• At least two (2) years clinical experience in a direct service provision to children and adolescents in a mental health setting preferred.
• Knowledge of the community resources as well as health and social service systems in the Kansas City metro area and skill in establishing working relationships with community partners preferred.
• Excellent customer service skills with a recovery focus.
• Strong written, oral and interpersonal communication skills.
• Intermediate computer skills in typing/keyboarding, Microsoft Word and Excel programs as well as the use of email.
