Job summary

ReDiscover is seeking a Community Support Specialist to serve as a frontline member of a mission-driven, nonprofit mental health program in the Kansas City area. You’ll manage a diverse caseload, help clients meet long-term goals, and coordinate with partners to connect individuals to needed services.

This role emphasizes compassionate, recovery-focused service delivery, strong communication, and reliable transportation to support client needs in the community.

Qualifications

• 4 year degree or 4 years equivalent experience.
• Valid driver’s license and a reliable vehicle for transporting clients.
• At least 2 years clinical experience with children/adolescents in a mental health setting preferred.
• Knowledge of Kansas City metro community resources and health/social service systems preferred.
• Excellent customer service with a recovery focus.
• Strong written, oral and interpersonal communication skills.
• Intermediate computer skills: typing, Word, Excel, and email usage.

Responsibilities

• Maintain and interact with a caseload of clients in-person or by phone.
• Support clients’ goals in symptom management, employment, and independent living.
• Engage clients to access community resources and services.
• Help clients overcome barriers and connect with educational or work programs.
• Assist with transportation as outlined in the treatment plan, including driving when needed.
• Document client interactions in the designated electronic medical record.

About The DM3700 Program

The Disease Management (DM) 3700 Program aims to outreach Medicaid-eligible adults who have a serious mental illness or substance use disorder, high medical costs, and are not currently receiving behavioral health services. The goal is to outreach and enroll individuals in services to reduce costs associated with care and assist individuals in accessing needed health services and supports. This program works in conjunction with CPRC and Healthcare Home in providing education to clients on how to manage their health conditions, improve general health, monitor conditions and intervene when health conditions are not properly controlled or managed.

What You’ll Do

• Maintain and interact with a caseload of clients with diverse characteristics either in-person or over the phone.
• Facilitate each client’s achievement of long range outcomes of symptom management, employment and independent living based upon the needs of the client.
• Engage difficult to serve clients by investigating all potential resources in order to gain access and make connections.
• Help remove barriers for clients, which may include helping clients participate in community or social service programs and referring clients to educational or working programs.
• Assist clients with transportation barriers as identified in their treatment plan. This includes transporting clients in the employee's personal vehicle to various appointments and area resources.
• Collect and maintain information about clients and document interactions within ReDiscover’s designated electronic medical record.
• Communicate and collaborate with an internal multi-disciplinary team to address various aspects of wellness and ensure positive communication regarding the coordination of client care.

Requirements Of The Position

• 4 year degree or 4 years of equivalent experience in lieu of degree.
• A valid automobile driver’s license, appropriate automobile insurance and a safe, reliable, registered automobile available for use during work hours which includes transporting clients in personal vehicle.
• At least two (2) years clinical experience in a direct service provision to children and adolescents in a mental health setting preferred.
• Knowledge of the community resources as well as health and social service systems in the Kansas City metro area and skill in establishing working relationships with community partners preferred.
• Excellent customer service skills with a recovery focus.
• Strong written, oral and interpersonal communication skills.
• Intermediate computer skills in typing/keyboarding, Microsoft Word and Excel programs as well as the use of email.
