Job summary

Monarch Healthcare in Greenwood, IN is seeking a Community Specialist to build positive relationships with individuals receiving services and their families in work, home, school, or community settings. The role supports clients in achieving personal goals, while providing relief to caregivers and ensuring safety and quality of care.

The position requires a High School Diploma, a valid USA driver’s license, and experience with adults with mental health diagnoses or developmental disabilities is

Qualifications

• High School Diploma is required.
• Experience working with adults with a mental health diagnosis and/or developmental disabilities is preferred but not required.
• Valid driver’s license (USA) is required.

Responsibilities

• Assess individuals for hopes, dreams and desires; provide supports to facilitate achievement.
• Protect and promote the rights of individuals and assist in advocacy and caregiver relief.
• Act as a resource on community services and supports addressing identified needs/goals.
• Supervise and educate individuals in work, home, school or community settings.
• Provide one-on-one support to meet emotional, physical and medical needs.
• Maintain a safe environment by following safety procedures.
• Document progress, goals, outcomes and incidents accurately.
• Assist in orientation, mentoring and training of staff and substitute as needed.
