Job summary

Open Sky Community Services in Worcester, MA is seeking a Community Services Navigator to deliver case management and care coordination for individuals with mental health needs who are at high risk for admission to a 24-hour treatment setting.

You will develop support plans, liaise with providers and families, arrange transportation, and participate in on-call rotations. A valid driver's license and at least two years of relevant experience are required.

Qualifications

• High School Diploma, GED or equivalent required.
• Valid driver's license with reliable transportation.
• At least 2 years' experience with homelessness, justice involvement, or serious mental illness.
• Excellent communication, documentation, time management and organization.

Responsibilities

• Develop a Support Plan with the member to guide services and assessments.
• Coordinate treatment with community agencies, families, and providers.
• Provide temporary transportation assistance to medical appointments.
• Participate in an On-Call rotation.
