Open Sky Community Services is seeking a Community Supports Navigator to provide case management and outreach to individuals with mental health challenges in Central Massachusetts. You’ll develop a Support Plan, complete assessments, and coordinate care with healthcare and community partners to reduce hospitalization risk.

You’ll act as liaison between members, families, and service providers, assist with transportation to appointments, and participate in an on-call rotation.

Formación

• High School Diploma, GED or equivalent required.
• Excellent communication, documentation, time management and organizational skills required.
• Valid Driver’s License with acceptable driving history and reliable, insured transportation.
• Minimum of 2 years’ experience or lived experience with homelessness, justice involvement, serious mental illness, substance use disorders, or underserved populations.

Responsabilidades

• Develop a Support Plan with the member that guides services.
• Complete all Assessments and Support Plans according to prescribed timelines.
• Act as a liaison between members, community agencies, and families to coordinate treatment.
• Work with medical and behavioral health providers to obtain active care plans.
• Provide temporary transportation assistance to essential medical/behavioral health appointments.
• Participate in an On-Call rotation.
