Job summary

North Los Angeles County Regional Center (NLACRC) is seeking a Service Coordinator for SFV School Age 12 to provide information, advocacy, and coordinated services for individuals and families. You will help develop and monitor plans, coordinate community resources, document requirements, and advocate for clients in collaboration with a multidisciplinary team.

The role requires strong communication skills, independence, and the ability to meet clients in homes or community settings.

Qualifications

• Bachelor’s degree in psychology, social work, sociology, or related human services field and at least two years of related experience.
• A Master’s degree in a related subject may be substituted for experience.
• Bilingual Spanish may be required dependent upon position.

Responsibilities

• Provide information, advocacy, and service arrangement for consumers and families.
• Develop program plans for individuals; monitor and revise as needed.
• Identify and coordinate services for individuals and their families.
• Document and complete forms and reports per NLACRC policies.
• Advocate for individuals served by community agencies.
• Schedule and attend interdisciplinary meetings with individuals served and others.
