Job summary

NLACRC is seeking a Consumer Services Coordinator for the San Fernando Valley. You will provide information, advocacy, and service arrangements for consumers and families, develop plans, coordinate services, and document per regulations.

The role involves teamwork with an interdisciplinary group, home/community visits, and on-call coverage when needed. Requirements include a bachelor’s in a human services field, at least two years of related experience (master’s may substitute), strong

Qualifications

• Bachelor’s degree and two years of related experience; a Master’s degree may substitute for experience.
• Understanding of family systems and person-centered planning.
• Knowledge of intellectual and developmental disabilities.
• Proficiency with MS Word and MS Outlook; strong written/verbal communication.

Responsibilities

• Provide information, advocacy, and service arrangement for consumers and families.
• Develop program plans for individuals; monitor and revise as needed.
• Identify and coordinate services for individuals and families.
• Document forms and reports per NLACRC policies and regulations.
• Serve as an advocate for individuals served by community agencies.
• Attend interdisciplinary meetings with individuals, families, and providers.
• Work in home, community, or office; participate in on-call duties when required.
