Job summary

JobAps, on behalf of the Caroline County Health Department, seeks a Coordinator to manage Targeted Case Management and CCS programs for individuals with developmental disabilities on Maryland's Eastern Shore.

You will assess eligibility, refer to DDA services, and coordinate access to day, residential and related supports, while developing Person-Centered Plans and monitoring outcomes to improve quality of life.

Qualifications

• Bachelor's degree in a Human Services field or higher.
• Experience coordinating health-related programs or referring to resources.
• Minimum one year of professional experience in related coordination.

Responsibilities

• Coordinate services for individuals through DDP Targeted Case Management and CCS programs.
• Assess eligibility and provide referrals for DDA services.
• Arrange access to day, residential and other support services.
• Develop Person-Centered Plans and conduct monitoring of PCPs.
• Maintain documentation and assist with transition planning.
• Follow waiting list registry and support family planning.
