Job summary

Mary’s Center seeks a Community Schools Coordinator to lead the community school model at Tubman Elementary School, bridging school and community supports. You will coordinate health, behavioral health, family engagement, and social services, while guiding partnerships and program development.

The role also supports budget development, grant reporting, and fundraising coordination; you will facilitate Community School Advisory Board meetings and monitor progress toward goals.

Qualifications

• Bachelor’s in Social Work, Health Education or other Human Services field; 1-3 years as case manager or clinical support preferred.
• Ability to work independently with attention to detail, accuracy, and quality awareness.
• Analytical and problem-solving skills.
• Strong written and interpersonal communication; well organized and able to multitask.
• Knowledge of patient confidentiality issues and HIPAA compliance.
• MS Office applications including Outlook, OneDrive, Teams, Word, Excel, PowerPoint.

Responsibilities

• Develop and implement a needs assessment survey and community school plan.
• Provide case management to connect students and families with health, housing, food, employment, and other resources.
• Coordinate family-engagement activities and partnerships with community organizations.
• Link students and families to Mary’s Center’s medical, dental, and mental health services.
• Plan and coordinate programs and events; maintain annual calendar; manage logistics and outreach.
• Monitor progress toward goals and prepare mid-year/end-year reports for funders.
