Job summary

Responsibilities

• Facilitates case management of participants following program guidelines; develops rapport and case plans; tracks progress and coordinates referrals.
• Maintains relationships with community agencies to promote services and coordinate cases; ensures releases of information are on file.
• Creates case plans and connects participants with agency resources and external referrals as appropriate.
• Participates in educational forums to enhance knowledge and skills.
• Maintains accurate case records in electronic databases and monitors service transactions and assessments.
