Job summary

The Salvation Army is seeking a Case Manager to provide relational support to low-income clients, track case status, and help participants move toward family sufficiency. You will coordinate referrals, document progress, and maintain ethical standards in service delivery.

Responsibilities include collaborating with community partners, managing case plans, and ensuring confidentiality. A Bachelor’s degree and 2+ years in case management are preferred.

Qualifications

• Bachelor's degree or equivalent with knowledge in social services.
• Minimum 2 years of applicant interview or case management experience.
• Experience with program target population preferred.

Responsibilities

• Direct case management support and goal tracking for participants.
• Develop case plans, coordinate referrals, and monitor progress.
• Maintain partnerships with community agencies and ensure releases of information.
• Document case records using electronic databases per protocol.
• Engage in ongoing training and contribute to reports.
