Erie Neighborhood House seeks a Community Resource Coordinator to connect individuals and families with essential services, promote stability, and support self-sufficiency. You will coordinate community-based programs, provide case management, and navigate resources with participants and partners.

The role involves home visits, workshop coordination, data management for grants, and building strong collaborations with schools, healthcare providers, and community organizations.

Qualifications

• Bachelor's degree in social work, psychology, human services, or related field.
• Fluency in English and Spanish.
• Experience providing direct services in community-based settings.
• Experience with case management, resource navigation, and participant advocacy.
• Strong organizational, communication, and time management skills.
• Ability to manage multiple projects with accurate documentation.
• Knowledge of trauma-informed and culturally responsive practices.
• Commitment to social justice and serving diverse communities.

Responsibilities

• Provide case coordination, resource navigation, and referrals for participants.
• Support enrollment and connect individuals to internal and external resources.
• Coordinate workshops, groups, and parent education sessions.
• Conduct home visits to support engagement and service delivery.
• Develop and maintain partnerships with community organizations.
• Maintain participant records and assist with grant reporting.
• Represent the agency at community and funder meetings.
• Participate in trainings and contribute to quality improvement.
