The Community Resource Coordinator plays a key role in connecting individuals and families to services that promote stability, well-being, and self-sufficiency. This position coordinates community-based programming, provides case management and resource navigation, supports grant and program operations, supervises assigned staff and interns, and strengthens partnerships with community organizations.Working closely with participants, Erie Neighborhood House staff, and external partners, the Community Resource Coordinator helps ensure high-quality, trauma-informed services while supporting program growth, compliance, and community engagement.Participant ServicesProvide comprehensive case coordination, resource navigation, and referrals for participants and community members.Support the completion of the enrollment process and identify the needs and connect individuals and families to appropriate internal and external resources.Support walk-in participants and respond to referrals in a timely and compassionate manner.Conduct home visits when appropriate to support participant engagement and service delivery.Program CoordinationCoordinate, facilitate, and survey workshops, support groups, parent education sessions, and community presentations.Collaborate with Adult & Family Programs staff to deliver high-quality, participant-centered programming.Provide technical assistance and educational support to participants, staff, and community partners.Data Management & Grant ComplianceMaintain accurate participant records and enter required information into agency databases.Collect and organize program documentation, including attendance records, surveys, referrals, and other funder-required materials.Monitor referral activity and support grant reporting, program evaluation, and performance tracking.Ensure timely, accurate, and confidential documentation in accordance with agency and funder requirements.Community EngagementBuild and maintain partnerships with community organizations, schools, healthcare providers, and other stakeholders.Plan, coordinate, and participate in outreach events, recruitment efforts, and community collaborations.Represent Erie Neighborhood House at community meetings, funder meetings, collaborative initiatives, and public events.Other ResponsibilitiesParticipate in agency meetings, trainings, and professional development opportunities.Support continuous quality improvement initiatives across Adult & Family Programs and Community Wellness Support ServicesPerform other duties as assigned to support the mission and goals of Erie Neighborhood House.Success In This Role Demonstrate:Strong relationship-building and community engagement skills.Excellent organization, follow-through, and attention to detail.Compassionate, participant-centered service delivery.Commitment to trauma-informed, culturally responsive practices.Ability to manage competing priorities while maintaining high-quality services.Dedication to Erie Neighborhood House's mission, vision, and values.WORK ENVIRONMENT:This position works in both office and community-based settings and requires regular interaction with participants, families, community partners, and Erie Neighborhood House staff.Responsibilities include travel between Erie Neighborhood House locations, participant homes community organizations, and outreach events.
