Job summary

Union Community Care in Lancaster, PA seeks a Community Health Resource Coordinator to coordinate social services with medical and dental teams and advocate for patient-centered, integrated care. The role focuses on helping vulnerable patients access social supports and health resources.

You will support enrollment in public and private health insurance, assist with housing and other benefits, and translate or interpret as needed in a diverse community setting.

Qualifications

• Bachelor's degree preferred or HS diploma with relevant experience.
• 3+ years of relevant experience.
• Certified Application Counselor or Navigator required.
• Excellent communication and ability to document in the Electronic Health Record (EHR).

Responsibilities

• Serve as an active member of medical and dental teams to coordinate social services referrals.
• Collaborate with individuals and families to create care plans based on trust and cultural humility.
• Support and promote social assistance, including health insurance, housing, mental health, and other benefits.
• Enroll adults, children and families in health insurance programs.
• Conduct outreach to promote health insurance access for families and individuals.
• Provide services in the language most comfortable for the patient/consumer.
• Represent Union Community Care in public venues and outreach events.
• Document referrals and communications in the EHR.
