Job summary

Endeavor Health Services seeks a Social Determinants of Health Navigator to conduct screenings, connect individuals with community resources, and coordinate care across providers and internal staff. You will meet clients in person or virtually, document findings in the Findhelp platform, and strive to meet monthly productivity targets.

Qualifications

• Experience in case management, care coordination, community resource navigation, or a related social services role.
• Proficiency with Microsoft Office applications.

Responsibilities

• Conduct Social Determinants of Health (SDoH) screenings using a standardized assessment tool.
• Provide person-centered navigation services to connect individuals with community resources and support services.
• Meet with individuals in person, by phone, or through secure virtual platforms.
• Identify barriers related to housing, food insecurity, transportation, healthcare access, and other social needs.
• Coordinate referrals and collaborate with healthcare providers, community organizations, and internal staff to ensure seamless, coordinated care.
• Maintain accurate and timely documentation in the Findhelp platform and internal systems.
• Meet or exceed monthly productivity expectations of 80 billable SCN service hours.
• Participate in regional trainings, meetings, and quality improvement initiatives related to the NYS 1115 Waiver.
• Provide coverage for fellow Navigators as needed and participate in agency committees and meetings.
