Job summary

Tri-City Mental Health Authority in Pomona is recruiting a Community Navigator to support the Navigator Team within the Housing Services Department. You will perform community-based duties, including outreach, referrals, and linking individuals to housing and behavioral health resources.

The ideal candidate has an associate degree or relevant coursework and at least two years of outreach experience. Bilingual in Spanish preferred but not required; bilingual candidates encouraged.

Qualifications

• Two (2) years of experience providing outreach to a diverse community for health or behavioral health programs, or a related field.
• Equivalent to an associate degree with college-level coursework in community health, behavioral health, or a related field.

Responsibilities

• Collaborates with agencies to outreach, engage, and refer individuals to services and housing programs.
• Provides outreach and information on community resources and referrals; links individuals to agencies and health providers.
• Recruits community-based organizations and faith-based groups to build a local support network for people with behavioral health needs.
• Performs home, shelter, or street visits to provide information and education; supports unsheltered individuals and advocates for tenant needs.
• Identifies and provides transitional housing options and helps tenants contact property managers to prevent evictions.
