Job summary

Qualifications

Responsibilities

• Collaborate with service agencies to outreach, engage, and refer individuals to housing and health services.
• Provide information on community resources and connect clients to appropriate agencies and networks.
• Visit individuals in homes, shelters, or other settings to provide information, education, and advocacy.
• Identify transitional housing options and support tenants in interactions with property managers.
