Job summary

Community Action Partnership of Orange County (CAP OC) is seeking a Community Health Worker to support Housing Navigation, Enhanced Care Management, and Family Resource Centers. You will guide participants through health and social services, with a focus on homeless and high-utilizer populations, helping them access essential resources.

The ideal candidate is bilingual (English/Spanish), skilled in navigation, advocacy, and culturally sensitive care.

Qualifications

• Bilingual in English and Spanish required, both written and oral.
• Experience with homeless, housing services, or poverty-related programs is preferred.
• Knowledge of Medi-Cal and CalFresh processes is helpful.

Responsibilities

• Engage with the community through outreach and help clients access case management services.
• Collaborate with partner programs and CAP OC services for participants at FRCs and CAP OC sites.
• Assist with front desk duties, program activities, health education, and service provision.
• Support Housing Navigation and ECM by aiding with forms, applications, intakes, and coordination of services.
• Provide culturally appropriate health education and connect participants to community resources and referrals.
