Job summary

Community Partners NH is seeking a Resource Center Specialist to connect individuals to services and resources across health, housing, financial aid, and community supports. The role emphasizes helping people access disability-related services, medical programs, and legal or vocational resources to empower independent living.

The candidate should have strong communication, customer relations experience, and be comfortable using technology; a valid driver's license is required.

Qualifications

• High School Diploma or GED required.
• Minimum of 3 years social work or case management experience in aging, disabilities, community health, nursing home or hospital discharge planning preferred.
• AIRS certification or commitment to become certified within one year of hire.
• Excellent oral and written communication skills.
• Experience in customer relations, call management, information and referral or related field.
• Ability to utilize technology provided to perform requirements of the job.
• Hold a valid driver’s license and proof of vehicle liability coverage.

• Connect individuals to services and resources to meet their needs.
• Assist with linking to financial assistance, medical programs, housing, food, or other services.
• Support clients to live independently within their communities.
