Job summary

Easterseals New Hampshire is hiring a Community Resource Specialist to educate and advocate for NH residents, helping them navigate long-term services and supports and access community resources. The role involves outreach across offices in Manchester and Nashua, with emphasis on unbiased, person-centered guidance.

The position is full-time (40 hours/week) with typical hours Mon-Fri, and compensation of $21/hour.

Qualifications

• Bachelor’s degree in Social Work, Education, Human Services, Sociology, Cultural Anthropology, or Behavioral Science, OR five years’ experience in programs serving elderly or disabled.
• Knowledge of community long-term care services for elderly/disabled population.
• Knowledge of Medicaid and Medicare programs.
• Strong verbal, written and interpersonal skills.
• Basic computer skills (MS Office and databases).
• Valid driver’s license, auto insurance, and reliable transportation.

Responsibilities

• Assess caller’s situation and provide referrals to long-term supports and services via phone, walk-ins, emails, home visits, and community settings.
• Provide information on and referrals to community resources meeting needs.
• Offer long-term support counseling in hospitals, rehab facilities, and nursing homes.
• Complete eligibility screening and assist with understanding and applying for state/federal programs.
• Document contacts in the assigned database per contract and InformUSA protocols.
