Job summary

Easterseals NH & VT is hiring a Community Resource Specialist to educate, advocate, and assist NH residents in navigating long-term services and supports. The role focuses on unbiased, person-centered guidance to maintain independence and dignity across community organizations. The position is full-time with hours Monday–Friday.

Travel between Manchester and Nashua offices is required. Compensation is $21/hour with a comprehensive benefits package including health, paid time off, retirement

Qualifications

• Bachelor’s degree in Social Work or related field or five years of relevant experience in elderly/disabled services.
• Knowledge of community long-term care services for elderly/disabled populations.
• Strong written and verbal communication; interpersonal skills.
• Ability to perform home visits and community outreach; basic computer skills.

Responsibilities

• Assess contact/caller needs and refer to long-term supports and services via phone, email, in-person, or home visits.
• Provide information and referrals to community resources meeting needs of clients.
• Offer long-term support counseling in hospitals, rehab facilities, or nursing homes.
• Complete eligibility screenings and explain state/federal programs; document all contacts per protocols in the database.
• Follow InformUSA and ADRC contract reporting requirements; perform follow-ups as required.
• Provide Medicare information and counseling during the Annual Enrollment Period.
• Attend mandated meetings and assist with general office tasks as needed.
• Obtain required certifications (InformUSA AIRS; SHIP Counselor) within 6–12 months of hire.
