Qualifications

• High school diploma or GED.
• 3 years of experience in community agency providing direct care services to clients.
• Valid driver’s license when driving any vehicle for work-related reasons.
• Microsoft software skills with programs such as PowerPoint, Excel, Word, and Access.
• 4-year degree in health care, social services, or related field to be preferred.

Responsibilities

• Link community resources to patients in need and address social determinants of health.
• Assess and triage patient social service and community resource needs.
