Job summary

La Clinica is seeking a Community Resource Specialist to connect patients with community resources addressing social determinants of health. You will document care in EPIC and follow engagement workflows to meet program standards.

The role involves advocating for patients, teaching self-advocacy, and occasionally meeting patients in the community to support access to resources. Strong bilingual English/Spanish skills are required.

Qualifications

High school diploma or GED required.
Completes Community Health Worker certification within 6 months of employment.
Excellent interpersonal communication and problem-solving skills.
Ability to intervene and promote reconciliation, compromise and positive outcomes.
Bilingual in English/Spanish required.

Responsibilities

Screens/assesses for social determinants of health (SDOH) needs and connects patients to resources within the community.
Provides relevant community and/or health care resources, assisting patients in navigation to those resources.
Documents patient care in EPIC and follows workflows to meet patient engagement standards.
Advocates for patients and teaches self-advocacy strategies.
Participates in community events to provide La Clinica information.
May meet patients in the community to assist accessing resources.
