Job summary

Union Community Care seeks a Community Health Resource Coordinator to help patients meet social and behavioral goals by coordinating with medical teams and social services. You will support enrollment in health insurance, housing, mental health referrals, and other benefits, while documenting all actions in the EHR.

The role emphasizes cultural humility, language access, and collaboration with community partners to reduce barriers to care and improve overall health outcomes.

Qualifications

• Bachelor's degree preferred, HS diploma accepted with relevant experience.
• Certified Application Counselor or Navigator, required.
• Strong communication and collaborative skills with communities and health teams.

Responsibilities

• Coordinate social assistance referrals with medical, dental, and behavioral health teams.
• Develop care plans with patients/families focusing on social determinants of health.
• Enroll patients in health insurance and assist with applications.
• Provide services in the patient’s preferred language and advocate across providers.
• Document referrals and communications in the Electronic Health Record.
