UnityPoint Health Meriter is seeking a Community Resource Specialist to connect patients with community resources and address social determinants of health. The role collaborates with clinical staff to assess needs and triage services, coordinating referrals across clinics as needed.

• 4-year degree in health care, social services, or related field to be preferred.

The Community Resource Specialist role is essential to provide UnityPoint Health Meriter medically homed patients with comprehensive care and work to address social determinants of health.

This position collaborates with other clinical staff to assess and triage patient social service and community resource needs and coordinate services and referrals as appropriate.

The primary role of the Community Resource Specialist is to link community resources to patients in need.
