Responsibilities

Screen and assess SDOH needs and link patients to community resources.
Help patients navigate access to resources and services as needed.
Document care in EPIC and follow APCM workflow to meet engagement standards.
Support CareSTEP initiatives: online portal engagement, accessing resources, transportation, coordinating care.
Advocate for patients and teach self-advocacy strategies.
Staff community events to share La Clinica services information.
Occasionally meet patients in the community to support accessing resources.
Update resource information with Community Resource Team Lead.
