Maricopa County is seeking a Community Reentry Planner to support justice-involved individuals reentering the community after incarceration. Qualifications
- One year of full-time behavioral health work and/or substance use experience.
- Bachelor's degree in Behavioral Health, Social Services, or a related field.
- Bilingual skills in English and Spanish.
Responsabilités
- Provides case management and behavioral health navigation services for justice-involved individuals to assist reintegration.
- Develops and coordinates community re-entry plans addressing recidivism risk factors.
- Coordinates with a multidisciplinary team to develop discharge plans including community entities.
