Job summary

Correctional Health Services seeks a Community Reentry Planner to assist justice-involved individuals reentering the community from Maricopa County jails. You will screen for OTP, educate on OUD medications, and connect patients to community clinics and needed services.

The role emphasizes collaboration within the CHS Community Reentry Division, with integration to the Opioid Treatment Program and AHCCCS processes, aiming to support successful reintegration and recovery.

Qualifications

• One year of full-time behavioral health work and/or substance use experience required.
• Bachelor's degree in Behavioral Health, Social Services, or related field required.

Responsibilities

• Screen patients for the Opioid Treatment Program (OTP).
• Provide education on medications that treat Opioid Use Disorder (OUD).
• Facilitate connections to community clinics for ongoing support after release.
• Submit referrals to ensure patients receive needed care and services.
