Job summary

Maricopa County is seeking a Community Reentry Planner to support justice‑involved individuals reentering the community from county jails. You will screen patients for the OTP, educate on medications for OUD, and coordinate connections to community clinics for ongoing care after release.

Qualifications

• One year of full‑time behavioral health work and/or substance use experience.
• Bachelor's degree in Behavioral Health, Social Services, or a related field.

• Provide education on medications that treat Opioid Use Disorder (OUD).
• Facilitate connections to community clinics for ongoing support after release.
