Job summary

Tempus Unlimited is seeking a Community Based Personal Care Management Skills Trainer for our Personal Care Attendant Program in Plainville, MA. This role covers Plainville and up to a 60-mile radius, requiring travel, a valid driver’s license, and a reliable vehicle.

The position is full-time (35 hours/week), Monday to Friday, 8:30 am to 4:30 pm, with bilingual English preferred. Training will occur in Stoughton with a two-week orientation.

Qualifications

• Experience with disabilities and knowledge of PCA programs preferred.
• Teaching or counseling experience is helpful.
• Excellent communication, organizational, and writing skills.
• Basic computer skills in Microsoft Office; ability to navigate databases.
• Specialized training provided as needed.

Responsibilities

• Follow up on assigned functional skills training per contract.
• Assess consumer ability to manage PCA services.
• Train consumers/surrogates to manage their PCA program as outlined.
• Maintain confidentiality of consumer information.
• Attend in-service, supervision and staff meetings when requested.
• Demonstrate knowledge of Mass Health regulations and PCM Contract.
• Report suspected abuse/neglect to DPPC as required.
• Complete internal requirements and tracking documentation.
• Process referrals, service agreements, and assessments as needed.
