Job summary

Hillendale Home Care, Inc. in Walnut Creek is seeking a dynamic Home Care Community Partner to cultivate referral relationships and drive growth in the local market.

The role focuses on developing partnerships, coordinating referrals, and engaging the community to support long-term business growth. The position combines field-based business development with referral coordination and family engagement, collaborating with Intake, Client Care Managers, and Operations to ensure smooth transitions

Qualifications

• Three or more years of successful experience in business development, healthcare sales, physician relations, senior living, home care, hospice, community outreach, or another relationship-driven role.

Responsibilities

• Develop and maintain referral relationships with hospitals, skilled nursing facilities, senior living communities, physicians, fiduciaries, elder law attorneys, care managers, and other community partners.
• Conduct consistent field visits to build relationships, uncover opportunities, and maintain Hillendale’s visibility.
• Develop and manage a pipeline of strategic referral relationships.
• Identify new partnership opportunities throughout the Walnut Creek market.
• Represent Hillendale at networking events, community organizations, educational programs, and industry functions.
• Develop account plans for strategic referral partners and follow through on relationship-building activities.
