• Fluent in English; Spanish is a plus.

Responsibilities

• Educate potential patients about Upward Health’s services and enrollment into programs.
• Engage patients in conversations and enroll them into Upward Health’s programs.
• Manage a personal caseload and perform outreach via phone and in-person visits.
• Collaborate with Outreach Manager and team to ensure successful outreach efforts.
• Document outreach activities and outcomes in company systems.
