Job summary

Upward Health, Inc. is seeking a Community Outreach Specialist to establish our presence in the community and enroll potential patients into our programs.

The COS educates individuals about our offerings, engages them in conversations, and guides enrollment, primarily via phone outreach with occasional in-person visits and other outreach strategies. The COS reports to the Outreach Manager and works with the team to ensure successful patient outreach and engagement.

Qualifications

• Strong verbal communication and persuasive abilities.
• Excellent interpersonal skills with the ability to build trust.
• Strong organizational and multitasking skills to manage a caseload.
• Self-motivated with the ability to work independently and meet outreach goals.
• Comfortable with fast-paced environments and adapting outreach methods.
• Proficient with computer systems for documentation and outreach management.
• Flexible with various outreach methods including phone and in-person visits.
• Fluent in English; Spanish a plus.

Responsibilities

• Educate patients about Upward Health’s programs and services.
• Engage individuals in meaningful conversations and enroll them into programs.
• Manage a personal caseload with phone outreach and in-person visits.

• Document patient information accurately in company systems.
