Job summary

Upward Health, Inc. seeks a Community Outreach Specialist to establish and expand the company’s presence in the community and engage potential patients. The COS educates individuals about services, initiates enrollment, and manages a personal caseload using phone outreach, with in-person visits as needed.

The role reports to the Outreach Manager and emphasizes relationship-building, adaptability, and compassionate patient engagement across diverse communities.

Qualifications

• Excellent verbal communication and interpersonal skills.

• Proficient with computer systems for documentation and outreach management.
• Fluent in English; Spanish proficiency is a plus.

Responsibilities

• Manage a personal caseload and conduct outreach primarily by phone.
• Conduct in-person visits and other community-based activities as needed.
• Educate potential patients about Upward Health’s offerings and programs.
• Guide eligible individuals through enrollment into Upward Health programs.
• Collaborate with the Outreach Manager and team to optimize engagement.
