UnitedHealthcare Community Plan of Kentucky is seeking a Community Outreach Coordinator to plan, implement, and evaluate engagement and education initiatives for its Medicaid program across Kentucky. This role requires building partnerships with community groups, health providers, and public agencies to improve access to care and health outcomes.

The position is field-based with up to 75% travel, including evenings/weekends; telecommuting is possible from Fayette County or surrounding areas.

Qualifikationen

• High school diploma or GED required.
• 1+ years in community outreach or related field.
• 1+ years developing and maintaining community partnerships.
• Proficient with Microsoft Office (Outlook, Teams, Excel, PowerPoint).
• Willing to travel up to 75% across Kentucky; reside in Fayette County or surrounding areas.

Aufgaben

• Develop, cultivate, and maintain strategic partnerships with community-based organizations, advocacy groups, schools, health care providers, local health departments, family resource centers, youth service centers, chambers of commerce, faith-based organizations, shelters, and other community stakeholders.
• Represent UnitedHealthcare Community Plan of Kentucky at community events, health fairs, educational forums, stakeholder meetings, conferences, coalition activities, and Quality Member Access Committee activities.
• Identify opportunities to expand community partnerships that improve member access to health care services, education, resources, and support programs.
• Serve as a trusted liaison between community stakeholders and the health plan, gathering feedback and market intelligence to identify emerging needs, barriers to care, and opportunities to improve member experience and health outcomes.
• Plan and execute member and community outreach activities that increase understanding and use of Medicaid benefits, preventive care, care management, behavioral health resources, value-added benefits, population health initiatives, and available community supports.
• Conduct community presentations and educational sessions in virtual and in-person settings and promote awareness of programs such as WIC, SNAP, smoking cessation, parenting resources, behavioral health and substance use disorder services, and transportation assistance.
