Job summary

Talentify in Panama City, FL is seeking a driven Community Liaison to build partnerships with physicians, hospitals, skilled nursing facilities and home care clients across Bay, Gulf, Calhoun, Washington and Holmes counties.

You will coordinate care between referral sources, clients and internal teams, perform in‑home visits, and nurture a growing referral network with a friendly, results‑oriented approach.

Qualifications

• Sales or business development experience preferred, but not required.
• Strong interpersonal and relationship‑building skills.
• Excellent communication and organizational abilities.
• Self-motivated with the ability to work independently and as part of a team.
• Healthcare experience is a plus, but not required.

Responsibilities

• Build and maintain strong relationships with referral sources, including physicians, hospitals, skilled nursing facilities, and assisted living communities.
• Promote and educate partners on home care services, clearly communicating value and identifying client needs.
• Serve as the primary liaison between referral sources, clients, and internal teams to coordinate care and ensure a smooth start of services.
• Maintain ongoing communication with partners to support satisfaction and strengthen long‑term relationships.
• Represent the company in the community through outreach efforts and in‑home visits with prospective clients.
• Monitor market feedback and referral activity, providing insights to improve growth and marketing strategies.

Key Responsibilities
