Zusammenfassung

Centerstone Indianapolis is seeking a School Based Navigator to maximize enrollment in comprehensive insurance coverage. The role will involve helping individuals with eligibility, facilitating enrollment, and building partnerships to expand outreach to uninsured populations.

You will serve as a liaison between health and social services, empower patients, and connect them to community resources while coordinating with internal teams to improve access and outcomes.

Qualifikationen

• High school diploma or GED with some college is required.
• Bachelor’s degree in Social Work, Psychology, Public Health Nursing, Health Education, or a related field from an accredited school is preferred.
• Two (2) years of experience in community health, human services outreach and/or healthcare preferred.
• Background check, fingerprinting, Relias training and driving records may be required.

• Help individuals with insurance eligibility options and enrollment.
• Engage in community outreach to recruit uninsured patients.
• Serve as liaison between health/social services and the community.
• Connect patients to community resources and internal services.
• Maintain tracking of patient engagements and enrollments using Microsoft Excel/Google Sheets.
