Job summary

Heartshare in Brooklyn, NY is seeking a Full-Time Day Hab Specialist to implement Day Services Plans and help participants achieve their goals in center and community settings.

You will train participants, support inclusion, and transport clients as needed, with a focus on safety and data accuracy. A high school diploma or GED is required, along with a NYS driver's license; CNA/DSP credentials are preferred. Qualifications

• High school diploma or GED required.
• Valid NYS driver's license required.
• CNA certification or Direct Support Professional certificate preferred.
• One or more years' experience with individuals with developmental disabilities preferred.

Responsibilities

• Implement Day Services Plan and participate in activities at the program center and in the community.
• Train participants to develop skills, socialization, and active participation.
• Facilitate inclusion in the community and support relationship-building.
• Transport participants in agency vehicles as needed.
• Assist with group meals, toileting, and accompanying participants to clinics or emergency visits.
• Provide crisis intervention and report incidents to management promptly.
• Complete written reports and ensure participant safety and program orderliness.

Education

High School Diploma or GED

Valid NYS driver's license

CNA certification or Direct Support Professional certificate
