Job summary

Qualifications

• High school diploma or equivalent.
• Must complete CHW program within six months of hire.
• One year clinical experience preferred.
• Current CPR certification or ability to obtain within 90 days.
• Driver license and eligibility for tribal vehicle insurance.

Responsibilities

• Participate in community health outreach activities.
• Advise clients on self-care, such as diabetes management.
• Identify high-risk members with overdue health needs.
• Distribute culturally appropriate educational materials.
• Attend community events to strengthen ties with health services.
• Provide case management and referrals to resources.
