Job summary

Scenic Bluffs Community Health Centers is hiring multiple full-time Community Health Workers to support clinics in Cashton and surrounding areas. CHWs help patients identify and overcome non-clinical barriers, navigate social services, and coordinate with care teams to ensure services are accessed.

Strong communication and outreach skills are essential. The role emphasizes ongoing navigation, outreach, education, and follow-through for patients with barriers, with travel to assigned clinic

Qualifications

• High school diploma or equivalent required.
• Experience in community health, patient navigation, or related fields preferred.
• CHW training/certification preferred, or willingness to complete training after hire.
• Spanish/English bilingual skills are helpful but not required.

Responsibilities

• Engage patients, identify barriers to health, and connect to services.
• Follow up to ensure needs are addressed and escalate urgent concerns.
• Coordinate with care teams to provide non-duplicative support.
• Document contacts, referrals, and outcomes in approved systems.
• Participate in outreach events and community activities.
