CommUnityCare seeks a Community Health Worker to provide culturally appropriate health education and help community members access needed care. The role collaborates with the care team to support patient-centered care and strengthens connections between patients, families, and health services.

Responsibilities include cultural mediation, goal setting, and facilitating referrals to social services. A valid Texas driver’s license and CHW training or certification within six months are required.

Competenze

• Minimum one year of experience in a community health setting.
• Certified Community Health Worker (CHW) or completion of a 160-hour training within six months.
• Valid Texas Driver’s License with access to a personal vehicle.
• Proof of automobile liability coverage (30/60/25) required.

• Collaborate with care team to provide patient-centered care.
• Provide cultural mediation between community members and CommUnityCare.
• Empower patients and families to engage in their care management.
• Help patients set personal goals and attend appointments.
• Coach patients in self-care and health condition management.
