Job summary

COPE Health Solutions seeks a Community Health Worker to support patients and families in navigating services, accessing resources, and adopting healthy behaviors within our Enhanced Care Management program. The CHW collaborates with providers and case managers to promote comprehensive, coordinated care.

Qualifications

• Valid California driver's license.
• High school graduate or equivalent; associate degree preferred.
• CHW formal training program preferred.
• Written and oral fluency in English and Spanish preferred.
• Experience in multicultural settings.
• 1–2 years in a community-based setting preferred.
• Basic computer skills; EMR experience preferred.
• Understand the community served and its resources.
• Strong communication, listening, and patient engagement skills.
• Willing to learn across cultures and norms.

Responsibilities

• Educate members about ECM services and enrollment; liaison between member and services.
• Support navigation of health care system and self-care management.
• Establish trusting relationships with patients and families.
• Provide follow-up, motivational interviewing, and goal setting.
• Bridge conversations and conduct face-to-face outreach for scheduling and needs assessment.
• Meet members in clinic, facility, or home to identify social determinants of health.
• Collaborate with care team to create individualized, culturally appropriate care plans.
• Assist with accessing health services and community resources.
• Facilitate communication among patients, families, staff, and community organizations.
• Document interactions in EMR within 24 hours; manage caseload.
• Maintain HIPAA compliance and confidentiality.
