Chapa-De Indian Health is seeking a Community Health Representative to connect patients and families with care, education and resources in our care coordination team. You will build trusted relationships and help patients navigate the health care system, coordinating services and referrals.
The role is hourly, with a schedule of Mon-Fri 8:00 AM–5:00 PM, and some flexibility to work at the Rocklin clinic 1–2 days weekly and extended hours as needed.

Join Chapa-De's Care Coordination team as a Community Health Representative, where you'll help connect patients and families with the care, education, resources, and support they need. You'll build trusted relationships within the community, help patients navigate the health care system, coordinate services and referrals, and serve as an important link between patients, families, community partners, and health care providers.
Hourly Pay: $27.35-$28.35
Schedule: Monday- Friday 8:00 AM - 5:00 PM - Flexibility to work at our Rocklin clinic 1-2 days per week and extended-hour shifts as needed.
