Job summary

Sun River Health is seeking a Wellness Liaison to coordinate outreach and enrollment into Health Home and SRH services. The role requires building relationships with clients, providers, and community partners across Health Center and Health Home programs.

You will conduct eligibility screenings, arrange transportation, assist with insurance applications, and ensure accurate data entry in eCW, TREAT, and Salesforce-CRM.

Qualifications

• High school diploma or GED with 2-4 years in customer service.
• Bachelor preferred.
• Strong interpersonal and outreach abilities.

Responsibilities

• Participates in outreach and educational events.
• Serves as liaison between clients, providers and staff.
• Provides outbound reminder calls to clients.
• Assists clients in scheduling SRH Health Center services.
• Conducts Health Home pre-eligibility screenings and intakes.
• Achieves intake targets and maintains accurate data entry.
• Helps with transportation arrangements and insurance applications.
• Maintains HIPAA confidentiality and participates in team meetings.
