Sun River Health is currently seeking a full-time Wellness Liaison to join our team in the Poughkeepsie.
Wellness Liaison is responsible for providing outreach and education services to coordinate intake and enrollment into Health Home and all Sun River Health service lines and programmatic services. The position requires strong interpersonal skills to make connections and build relationships with the target patient/client base. The Wellness Liaison works collaboratively with other departments as well as community-based agencies with the purpose of making linkages to health services for community members. The Wellness Liaison will work externally at Community Partner sites, health fairs and events or may be based in the SRH centers. The Wellness Liaison is an Ambassador of SRH whose activities reflect the mission and value of the organization and will provide outstanding customer service.
High School Diploma/GED required with an additional four years of relevant experience in field of customer service in appropriate service industries. Bachelor Preferred.
